Two. One run can cover almost every item, except for some questline rewards. One such example is the mid-quest decision with the Gascoigne's Daughters side quest. For certain items, you need to send the young girl to Oedon Chapel, but also to Iosefka's Clinic.
Expect a mid-sized completionist journey taking about 35 hours for seasoned Soulsborne completionists and 50 hours for new players into the genre.
7/10. The game doesn't have that many items. Even if you miss something, if you already know the way, it is quick to get back to that area again.
Experienced Soulsborne players will only find the lack of Lamps (bonfires) a challenge - bosses are generally easy, builds straightforward and there are only few enemy types.
We would rank this 6/10, if not for Chalice Dungeons, which add a large difficulty spike and about an 8-hour time sink at the end to get all Chalices.

You should completely exhaust every NPC before defeating each major story boss to ensure not missing anything. Follow the Quests Checklist for more information.
The last possible moment to explore and collect items is before starting the Gehrman, the First Hunter boss fight. Beating the final boss will directly throw you into New Game Plus with no option to stay in the current run.
No, but there are exclusive items in questlines, so you'll need to go to NG+ anyway.
Dark Souls 1, Lies of P and to an extent all other Soulsborne games. No exclusive items in New Game Plus, linear paths in a small world with few optional areas. Missable and cryptic NPC quests. No need to grind multiplayer extensively.

What kind of data does the progress tracking functionality collect?
This page only displays your progress. The checklists you use send database keys for checked items plus a timestamp. That is tied to your user ID and account email. Sign-in and storage use a cloud database.
