Three. Two runs are needed to obtain all Music Records and Boss Amulets & Weapons, but certain Amulets only appear in New Game Plus Plus.
It will take you around 35 hours to beat Lies of P with all possible items in the first run.
New Game Plus is significantly faster and will depend on how much optional content you want to visit.
Full 100% Completion will take about 60 hours, if you are focused, skilled and can run through the Chapters without problems.
With Overture DLC: 15 hours for first playthrough, 7 for next playthroughs.
7/10. Most of the game is just picking up items from the ground and chests.
The biggest problem is the large amounts of missables and quest-dependent items, plus the requirements to go to New Game Plus and New Game Plus Plus.
The game is difficult and well-balanced, there is not any overpowered build or glitch to exploit. Your best option is to simply learn the layout of the areas and the attack patterns of the bosses.
After you become confident in combat, try to prioritize Dashing instead of using Perfect Guard (parry). The vast majority of boss attacks can simply be dodged and instantly punished, even though the game leans heavily into parrying. The Fury attacks (red glow) can also be dodged most of the time, even though the game warns you it's not possible. Dodging most attacks is significantly faster than trying to learn boss patterns across many failed attempts.
You should aim to fully complete each chapter before defeating that chapter's Story Boss, which can break some nearby side quests.
You can also free-roam anywhere after the main story. New Game Plus is started with your input at the Hotel Stargazer, this game's bonfire. Note that one of the endings disables decryption of Cryptic Vessel, possibly barring your entrance to Hermit's Cave, one of the subareas.
Dark Souls 2, Dark Souls 3. Many items are related to convoluted and cryptic quests, NG+ and NG++ is required for 100% Completion and picking up items is often interrupted by difficult bosses which require multiple attempts, before you learn them perfectly.
Level exploration is also very similar: pre-made linear levels with some side alleys and some backtracking.

Can I delete my progress and start over?
Yes, but that is handled from your account and profile flows rather than this tracker page. Read the help on your Profile and settings for how to manage saved progress.
What kind of data does the progress tracking functionality collect?
This page only displays your progress. The checklists you use send database keys for checked items plus a timestamp. That is tied to your user ID and account email. Sign-in and storage use a cloud database.
Does this support DLC?
Lies of P: Overture DLC is included via the Overture DLC Checklist (163 items). Silksong: Sea of Sorrow will be supported at launch. The Witcher 3 and Kingdom Come: Deliverance 2: full DLC coverage is planned very soon. Hades II and Nine Sols: no DLC exists yet.
