All Endings Checklist Overview
The checklist is located below this introductory text.
Bloodborne offers three endings, out of which one can be viewed as the true ending. The endings are decided by two things: your answer to 
Gehrman after finishing the 
Nightmare of Mensis area, and whether you've found and consumed at least three Thirds of Umbilical Cord. If you've successfully done the latter, this will lock you out of any other ending.
All endings are tied to unique trophies / achievements and are required for 100% Completion.
You should use the All Umbilical Cords Checklist to manage missables and not lose the option to get the true ending.

How many endings does Bloodborne have?
This checklist tracks 3 endings: Yharnam Sunrise, Honoring Wishes, and the true ending Childhood's Beginning.
How do I get the true ending?
Consume at least three Thirds of Umbilical Cord during the playthrough, then finish the game. That route overrides the other two endings. Track cords on the All Umbilical Cords Checklist.
Can I see more than one ending on one save?
No. Consuming three cords locks you into Childhood's Beginning for that playthrough. The other endings need separate runs without three cords consumed, then accepting or refusing Gehrman's offer.
Do endings matter for trophies?
Yes. Each ending has its own trophy / achievement, and all three are required for 100% Completion / platinum.
When is the ending choice made?
After finishing the Nightmare of Mensis area, you meet Gehrman in Hunter's Dream. Your answer, plus whether you consumed three cords, decides the ending.
